What Does a C-SNP Cover?
- All Original Medicare Benefits: C-SNPs must cover everything that Original Medicare (Parts A & B) covers.
- Prescription Drug Coverage: Most C-SNPs include Medicare Part D prescription drug coverage.
- Condition-Specific Benefits: Extra benefits and care coordination tailored to your chronic condition (for example, disease management programs, nutrition counseling, or access to specialists).

Who Can Enroll in a Chronic Medicare Advantage Plan?
You must:
- Be enrolled in Medicare Part A and Part B
- Live in the plan’s service area
- Have a diagnosis of one or more qualifying chronic conditions (such as diabetes, cardiovascular disorders, chronic lung disorders, etc.)

When Can You Enroll?
You can enroll in a C-SNP during:
- Your Initial Enrollment Period (when you first become eligible for Medicare)
- The Annual Enrollment Period (October 15 – December 7)
- A Special Enrollment Period (if you are newly diagnosed with a qualifying chronic condition)
